If you are coming in as a Patient, go up to the New Patient Forms tab, click on New Patient Forms to Print, print out the 5 pages, fill them out and bring with you to your appointment. This will save you time and get you started on your way to feeling better.
If you are coming in as an Acupuncture patient, please print out the 5 pages, and also the Acupuncture Consent form. Fill them out and bring with you to your appointment.
For those of you who have no insurance or limited coverage we offer cash discount at time of service.
If you are coming in for the diet, please print the 5 pages, and the pages under the SHAPE ReClaimed tab.
If you wish you can read the HIPPA information and print it. But we do not need those forms, they are only for you.
